Does Microneedling With Exosomes Really Work? What the Evidence Shows
Yes—but with an important qualification. And there is another distinction that is extremely important:
Microneedling devices and exosome products have different FDA regulatory statuses.
Certain microneedling devices have been legally authorized by the FDA for specific aesthetic uses, including improvement in the appearance of facial acne scars and facial wrinkles in appropriate patients. However, the FDA states that there are currently no FDA-approved exosome products, and the FDA has not authorized microneedling devices to deliver cosmetics, drugs, biologics or other products into the skin.

What Is Microneedling With Exosomes?

Microneedling is a procedure that uses very small needles to create controlled micro-injuries in the skin.
The goal is to stimulate a wound-healing response and remodeling of the skin.
Exosomes are a subset of extracellular vesicles released by cells. They can carry biological signals, including proteins, lipids and nucleic-acid-related molecules, that participate in communication between cells.
When the two are combined, the proposed rationale is that microneedling creates a controlled skin-remodeling response while an exosome or extracellular-vesicle preparation may provide additional biological signaling.
That mechanism is biologically plausible.
But biological plausibility is not the same thing as clinical proof. That distinction is important throughout the exosome literature.
A 2026 review of facial aesthetics found substantial preclinical and clinical interest in exosomes, including research involving collagen, elastin, inflammation, oxidative stress and tissue remodeling, but emphasized that high-quality randomized controlled trials remain limited.
Does Microneedling Alone Really Work?
Yes—and this matters when evaluating exosomes. It would be misleading to attribute every improvement following an “exosome microneedling” treatment to the exosomes.
Microneedling itself has evidence behind it.
A 2025 systematic review evaluated 21 studies involving 723 patients undergoing microneedling for facial rejuvenation. Wrinkles were the most frequently evaluated endpoint, followed by skin texture, photoaging and skin laxity.
The FDA has also legally authorized certain microneedling devices for specific aesthetic indications, including improving the appearance of facial acne scars and facial wrinkles in patients aged 22 and older.
Therefore, when researchers ask whether adding exosomes makes microneedling better, the relevant comparison is not:
“Exosomes versus nothing.” It is: “Microneedling alone versus microneedling plus an exosome/extracellular-vesicle preparation.”
That is a much harder—and much more clinically meaningful—question.

What Does the Research Say About Microneedling With Exosomes?

The newest systematic review is encouraging—but cautious. A 2026 systematic review specifically examined the use of microneedling combined with exosomes in dermatology. The authors concluded that the combination is promising but that larger studies and longer follow-up are needed. [2026 systematic review of microneedling combined with exosomes]
Researchers initially identified 256 unique references. After screening and quality appraisal, eight studies were included in the final narrative synthesis.
The studies involved sample sizes ranging from just 3 to 60 participants and examined conditions including:
- Skin aging
- Hyperpigmentation
- Scarring
- Enlarged pores
- Hair loss
The review concluded that microneedling and exosomes show promise, but emphasized that larger studies and longer follow-up are needed before the effectiveness and long-term safety of the combination can be established confidently.
This is one of the most important sources for answering the question posed by this article because it specifically evaluates the combination, rather than exosomes in general.
Is There Evidence That Exosomes Add Something to Microneedling?
Yes—there are signals that they may. But this is where the distinction between promising evidence and established evidence becomes important.
One prospective randomized split-face study evaluated 28 individuals with facial skin aging. Each participant received three treatment sessions. [View the randomized split-face study on PubMed]
One side of the face received microneedling combined with a human adipose tissue stem-cell-derived exosome-containing solution, while the other side received microneedling with saline as the control.
At final follow-up, the Global Aesthetic Improvement Scale score was significantly higher on the exosome-treated side. This type of split-face design is useful because each participant effectively serves as his or her own control.
However, the study was relatively small and follow-up was limited.
So it provides evidence of a possible incremental benefit, not definitive proof that exosomes always improve microneedling.
What Did a More Recent Microneedling Study Find?
A 2026 retrospective study evaluated 40 patients who received an exosome-containing skin booster together with microneedling.
Patients received four treatments at two-week intervals and were followed for eight weeks after the final treatment.
The investigators reported:
- 85% of patients scored “improved” or better on the Global Aesthetic Improvement Scale
- 87.5% overall satisfaction
- 82% intention to repeat treatment
- 85% willingness to recommend treatment
Reported adverse events were transient and mild, with no serious adverse reactions reported in that study.
Those results are encouraging. But there is an important limitation:
This was a retrospective study without a randomized control group.
Therefore, the study can tell us that patients appeared to improve following the treatment, but it cannot establish with the same confidence that the exosome component caused the improvement.
What About the Newest 2026 Randomized Study?
There is an especially interesting development because a randomized split-face study was published on August 24, 2026. The study evaluated 60 adults receiving radiofrequency microneedling. One side of the face received radiofrequency microneedling followed by a topical extracellular-vesicle preparation, while the opposite side received radiofrequency microneedling alone.
At 12 weeks, the EV-treated side showed greater improvement in wrinkle severity.
The investigators also reported numerical advantages for texture, pore visibility, erythema, pigmentation, hydration and Global Aesthetic Improvement Scale outcomes. Local reactions were transient and predominantly mild. This is important because it provides relatively recent controlled evidence that adding an extracellular-vesicle preparation may provide an incremental benefit over the procedure alone.
But even this study has limitations.
The investigators specifically noted:
- The study was unblinded
- Follow-up was only 12 weeks
- Histologic and molecular biomarkers were not included
Therefore, the results should be interpreted as a short-term efficacy signal, not proof of long-term regenerative effects. So, Does Microneedling With Exosomes Work Better Than Microneedling Alone?
The evidence suggests it might—but we cannot say that definitively yet. This is probably the single most important answer on this page. Some comparative human studies have found better outcomes when an exosome or extracellular-vesicle preparation is added to microneedling.
However:
- Studies are small
- Different products are used
- Different exosome sources are used
- Treatment protocols differ
- Follow-up periods differ
- Some studies are retrospective
- Some studies are unblinded
- Long-term data remain limited
The 2026 systematic review specifically examining microneedling plus exosomes concluded that more evidence is necessary before efficacy and safety can be established with confidence.
So the scientifically responsible answer is:
Microneedling with exosomes appears promising and may improve certain outcomes beyond microneedling alone, but the magnitude and consistency of that additional benefit have not yet been definitively established.
What Skin Problems Are Being Studied?

Research involving exosome-enhanced microneedling has explored several aesthetic concerns.
Fine lines and wrinkles
This is one of the most frequently studied outcomes.
Some studies report improvement in wrinkle appearance following combination treatment.
Skin texture
Studies have reported improvements in texture, including in comparative treatment designs.
Pigmentation
Hyperpigmentation and pigmentation-related measures have been evaluated in clinical research.

Can Microneedling With Exosomes Stimulate Collagen?
There is a biological rationale and supportive early evidence. Microneedling itself initiates a wound-healing and remodeling response. Exosomes may influence signaling pathways involved in extracellular-matrix remodeling. Preclinical research has reported effects involving collagen, elastin and fibronectin, while clinical studies have reported improvements in skin elasticity and appearance.
But there is an important caveat.
Showing increased collagen-related activity in laboratory research does not prove that a cosmetic treatment will produce a clinically meaningful amount of new collagen in every patient.
The 2026 literature continues to call for better standardized human trials.
How Quickly Do You See Results?

For example, the 40-patient retrospective study used four treatments at two-week intervals. The 2023 randomized split-face study used three treatment sessions separated by three weeks. That means treatment frequency should not simply be copied from one research study to every patient.

Does the Exosome Product Matter?
Absolutely.

Does “Exosome” Automatically Mean the Product Is FDA Approved?
No. The FDA states that there are currently no FDA-approved exosome products. Patients can review the agency’s current information about [FDA-approved exosome products] and the regulatory status of regenerative medicine products.
The FDA also regulates exosome products and has warned about unapproved products marketed to patients.
Therefore, terms such as:
- “FDA registered”
- “FDA compliant”
- “FDA facility”
- “GMP manufactured”
should not be interpreted as meaning:
“This exosome product has been FDA approved for cosmetic skin rejuvenation.”
Those are different regulatory concepts.
Is Microneedling FDA Approved?
The terminology matters. The FDA does not simply “approve microneedling” as one universal treatment. The FDA has legally authorized specific microneedling medical devices for specific uses.
According to the FDA, authorized microneedling devices include indications involving improvement in the appearance of facial acne scars and facial wrinkles in appropriate patients. The FDA also notes that microneedling devices have not been cleared for delivery of cosmetics, topical medications, drugs or blood products into the skin.
That distinction is extremely important when discussing microneedling with exosomes.
Is Microneedling FDA Cleared to Deliver Exosomes Into the Skin?
No.
This is where many online explanations become misleading.
The fact that a particular microneedling device has FDA authorization for an aesthetic indication does not mean the FDA has approved the device as a delivery system for exosome products.

Can You Tell From a Before-and-After Photo Whether Exosome Microneedling Works?
Not reliably. Before-and-after photographs can be useful for demonstrating a patient’s experience, but they cannot establish causation.
Appearance can change because of:
- Lighting
- Camera angle
- Makeup
- Hydration
- Skin care
- Natural healing
- Time
- Multiple treatments
- Image processing
Controlled studies are much more useful for determining whether a treatment actually caused an improvement. That is why randomized or split-face studies are particularly valuable.
Is Exosome Microneedling Better Than PRP?
There is currently not enough evidence to make a blanket statement that exosome microneedling is better than PRP. They are different biological approaches.
PRP uses components derived from the patient’s own blood.
Exosome products contain extracellular vesicles derived from a biological source that varies by product.
