What Causes Acne Scars?
Acne scars develop when inflammation from acne damages the deeper structures of the skin and the subsequent healing process produces an abnormal amount or organization of collagen.
The result may be:
- Depressed areas
- Uneven texture
- Irregular skin contours
- Changes in pigmentation
- Fibrotic or tethered areas
- Less commonly, raised scars
Not every mark left after acne is technically a scar.
Post-inflammatory hyperpigmentation
Dark or brown marks left after acne are often post-inflammatory hyperpigmentation (PIH) rather than true scars.
PIH is a pigmentary problem rather than a structural depression in the skin.
Atrophic acne scars
Atrophic scars are depressed below the surrounding skin because of loss or remodeling of dermal tissue.
They are commonly categorized as:
- Rolling scars
- Boxcar scars
- Ice-pick scars
These distinctions matter because different scar types can respond differently to treatment.
Can Microneedling Improve Acne Scars?
Yes. Microneedling is used clinically to improve the appearance of certain acne scars, particularly atrophic scars. The treatment creates controlled microscopic injuries in the skin.
This initiates a wound-healing response that can stimulate remodeling of the extracellular matrix and gradual changes in collagen organization.
The goal is not to “erase” the scar instantly.
Instead, repeated treatments may gradually make the transition between the scarred and surrounding skin less noticeable. The FDA has legally authorized certain microneedling devices for improving the appearance of facial acne scars in patients aged 22 and older. Importantly, this authorization applies to specific devices and specific intended uses—not to every microneedling device or every acne-scar treatment protocol.
Why Add Exosomes?
This is where the treatment becomes more experimental.
Exosomes are extracellular vesicles involved in cell-to-cell communication. Researchers are investigating whether their biological contents may influence processes involved in:
- Tissue repair
- Inflammation
- Cellular signaling
- Extracellular matrix remodeling
- Collagen-related pathways
- Wound healing
The theory behind combining microneedling and exosome-containing preparations is that microneedling provides a controlled remodeling stimulus while the exosome-containing preparation may provide additional biological signaling.
That is a plausible scientific hypothesis. But plausibility is not the same thing as proof.
The critical clinical question is: Does adding an exosome-containing preparation produce meaningfully better acne-scar improvement than microneedling alone?
The answer is not yet definitively established.
What Does the Research Show Specifically for Acne Scars?
The evidence is much smaller than the marketing surrounding exosomes might suggest.
A 2026 scoping review identified acne scars among the dermatologic conditions investigated in human exosome studies. The studies used different exosome sources and different delivery methods, including topical application following microneedling and other resurfacing procedures.
One particularly relevant 2025 case series examined three patients with active acne, post-inflammatory hyperpigmentation and atrophic acne scarring.
The patients underwent microneedling followed by topical application of a Lactobacillus-derived exosome preparation.
After two months, the investigators reported improvements in acne severity, pigmentation and atrophic scarring, with one-grade improvement in the Goodman and Baron scar score across the cases. Only transient erythema was reported as an adverse event.
However, there were only three patients, and there was no untreated or microneedling-only control group. That makes the study interesting—but not strong enough to establish effectiveness.
The Important Acne-Scar Study: Exosomes With Fractional CO₂ Laser

One of the better-known clinical studies involving exosomes and acne scars did not use microneedling. It used fractional CO₂ laser resurfacing.
In a 2020 prospective, double-blind, randomized split-face study, 25 patients underwent three fractional CO₂ laser treatments. One side of each face received an adipose-derived stem-cell exosome gel following laser treatment, while the other side received a control gel.
At 12 weeks, the exosome-treated sides demonstrated greater improvement in acne-scar scores than the control sides. The reported percentage reduction in ECCA scar scores was 32.5% versus 19.9%, respectively. The exosome-treated side also had milder erythema and shorter downtime.
This study is useful because it was randomized, double-blinded and split-face. But there is an important limitation:
It studied fractional CO₂ laser—not microneedling. Therefore, its findings cannot simply be presented as proof that exosomes improve microneedling results.
What About the Newer 2026 Research?
The exosome literature continues to evolve.
A 2026 randomized study evaluated fractional CO₂ laser combined with polynucleotide and exosome-mimetic nanovesicles in 100 patients with atrophic acne scars.
The combination group had greater improvement in ECCA scores and improvements in ice-pick, boxcar and rolling scar classifications compared with CO₂ laser alone. The study also reported fewer cases of post-inflammatory hyperpigmentation and prolonged erythema.
Again, however, this was an exosome-mimetic nanovesicle preparation combined with fractional CO₂ laser, not conventional microneedling with a standard exosome preparation.
That distinction is critical.
Evidence from related technologies can help establish scientific plausibility, but it should not be presented as direct evidence for every exosome-microneedling protocol.
So, Does Microneedling With Exosomes Actually Work for Acne Scars?
The most scientifically accurate answer is:
It may help, but we do not yet have enough high-quality evidence to know how much additional benefit exosomes provide beyond microneedling itself.
There are several reasons for this uncertainty.
1. Small studies
Some studies involve only a handful of patients.
2. Different products
“Exosomes” are not one standardized pharmaceutical product.
The source, manufacturing process, concentration and composition can differ substantially.
3. Different procedures
Studies have evaluated combinations with:
- Microneedling
- Fractional CO₂ laser
- Radiofrequency microneedling
- Other resurfacing procedures
Results from one procedure cannot automatically be transferred to another.
4. Different outcome measures
Researchers have used different scar grading systems, photography, patient satisfaction scores and instrument-based measurements.
5. Limited follow-up
Many studies follow patients for only weeks or a few months. Acne-scar remodeling can continue over longer periods, so longer follow-up is valuable.
Which Acne Scars Respond Best to Microneedling?

Microneedling is generally more relevant to atrophic acne scars than raised scars.
Rolling scars
Rolling scars have gently undulating depressions and may respond to treatments that stimulate collagen remodeling.
However, some rolling scars are tethered to deeper tissue. In those cases, subcision or another scar-release procedure may be more appropriate than microneedling alone.
Boxcar scars
Boxcar scars have relatively sharply defined edges and can vary considerably in depth.
Superficial boxcar scars may respond to resurfacing approaches, while deeper scars may require other procedures.
Ice-pick scars
Ice-pick scars are narrow and extend relatively deeply into the skin. Microneedling alone may not be the optimal treatment for many ice-pick scars. Other approaches, such as TCA CROSS or selected surgical techniques, may be considered depending on the individual scar.
Why Scar Type Matters More Than the Word “Exosome”
A common mistake is to look for one treatment that works for every acne scar. Acne scarring is not a single condition.
Two patients may both say they have “acne scars” while having completely different scar patterns.
One person may have:
- Mostly rolling scars
Another may have:
- Deep ice-pick scars
The treatment strategy should therefore be based on scar morphology, skin type, active acne status and the patient’s goals—not simply on whether a treatment is marketed as regenerative.
Microneedling With Exosomes for Acne Scars vs. Microneedling Alone
This is one of the most important comparisons.

The incremental benefit remains an active area of research.
What Does the FDA Say?
This section is particularly important because online marketing can make the regulatory situation confusing.
Are Exosomes FDA Approved?
No. The FDA states that there are currently no FDA-approved exosome products. Exosome products are regulated by the FDA, and products intended to treat diseases or conditions in humans generally require FDA approval.
Therefore, a product should not be described as an “FDA-approved exosome” merely because it is commercially available.
Is Microneedling FDA Approved?
The terminology matters.
The FDA has legally authorized specific microneedling devices for specific uses, including improving the appearance of facial acne scars in patients 22 and older.
It is therefore more accurate to say:
Certain microneedling devices have FDA marketing authorization for specific acne-scar uses.
It is not accurate to say:
“Microneedling is FDA-approved for everything.”
Has FDA Approved Microneedling With Exosomes?
No.
The FDA states that microneedling devices have not been cleared for use with another product, including cosmetics, topical medications, drugs, biologics or blood products such as PRP. The agency says that the safety and effectiveness of combining a microneedling device with another product have not been reviewed.
Consequently, FDA authorization of a particular microneedling device should not be interpreted as FDA approval of an exosome-plus-microneedling combination.
What About “FDA Registered” Exosome Products?
Patients should be particularly careful with this terminology.
Words such as:
- FDA registered
- FDA listed
- FDA compliant
- FDA certified
- Manufactured in an FDA-registered facility
do not mean that an exosome product has been FDA approved.
The FDA specifically states that there are currently no FDA-approved exosome products.
If a clinic advertises an exosome product as “FDA approved,” patients should ask exactly what the claim means and request the relevant FDA authorization information.
Is Microneedling With Exosomes Safe?
The available studies have generally reported mostly mild and temporary adverse effects when exosome-containing preparations were used topically in conjunction with resurfacing procedures.
However, the long-term safety profile is not fully established.
A 2026 scoping review found that adverse events in the clinical literature were uncommon but included more serious events such as granulomas, necrosis and allergic reactions in some exosome-based treatment contexts, particularly involving injection.
That does not mean those complications are expected with topical use after microneedling. It does mean that “exosome” should not automatically be equated with “risk-free.”
The specific product, route of administration, sterility, manufacturing quality and treatment technique all matter.
What Are the Risks of Microneedling?
Microneedling itself can cause temporary:
- Redness
- Swelling
- Burning or stinging
- Tightness
- Scaling
- Itching
- Sensitivity
More significant complications can occur, particularly if the procedure is performed improperly or on an unsuitable patient.
The FDA recommends that patients choose a healthcare provider with appropriate microneedling training and ask about device cleaning and use of a new cartridge for each patient and treatment session.
What About RF Microneedling?
Radiofrequency microneedling is different from conventional microneedling. RF microneedling combines needle penetration with delivery of radiofrequency energy into tissue.
In October 2025, the FDA issued a safety communication concerning certain uses of RF microneedling after receiving reports of serious complications, including burns, scarring, fat loss, disfigurement and nerve damage.
This does not mean that every RF microneedling treatment causes these complications. It does mean that patients considering RF microneedling should discuss the specific device, settings, treatment area, provider experience and risks.
Can Microneedling With Exosomes Treat Active Acne Too?
Some emerging research has examined exosome-containing preparations in patients who have both active acne and acne scarring. The 2025 three-patient case series involving Lactobacillus-derived exosomes and microneedling reported improvements in acne severity, pigmentation and atrophic scarring.
However, three patients are not enough to establish an acne treatment.
Active inflammatory acne generally deserves its own medical treatment plan. In many cases, it is preferable to control active acne before aggressively treating residual scarring.
What About Post-Inflammatory Hyperpigmentation?
Patients with acne scars frequently also have dark marks left behind from previous acne. These marks can sometimes become more noticeable after procedures that intentionally create inflammation.
This is particularly important for individuals who are prone to post-inflammatory hyperpigmentation.
The FDA specifically notes that darker skin types may have a risk of skin darkening or lightening after microneedling and that some FDA-authorized devices were not studied extensively in darker skin types.
A qualified provider should therefore evaluate both:
The structural scar and the patient’s pigmentation tendency.
How Many Treatments Are Needed?
There is no single universally established number of microneedling-with-exosome treatments for acne scars.
Treatment schedules vary according to:

- Smoother
- Less sharply defined
- More blended with surrounding skin
Results tend to develop gradually as collagen remodeling occurs. Some patients may notice improvement after a treatment series, while others require a combination of different procedures.
The amount of improvement depends heavily on the scar type.
When Might Microneedling Not Be the Best Choice?
Microneedling is not necessarily the optimal treatment for every acne scar.
A patient may need another approach when there are:
- Deep ice-pick scars
- Deep boxcar scars
- Strongly tethered rolling scars
- Raised hypertrophic scars
- Keloids
- Active inflammatory acne
- Significant pigmentation disorders
Depending on the individual, treatment may involve combinations of:
- Microneedling
- Subcision
- Chemical reconstruction techniques
- Fractional laser resurfacing
- Radiofrequency-based procedures
