A 2023 prospective randomized split-face study involving 28 participants compared microneedling plus an adipose-derived stem-cell exosome-containing solution on one side of the face with microneedling plus saline on the other. The exosome-treated side demonstrated significantly greater improvement on the Global Aesthetic Improvement Scale, along with improvements in measured wrinkles, elasticity, hydration and pigmentation. No serious adverse events were observed during the study.
That study is encouraging because each participant served as their own control.
However, it was still a relatively small study with a 12-week treatment/follow-up period.
2. Skin Texture

A 2025 split-face observational study involving nine women reported greater improvements in texture, hydration, elasticity and pigmentation on exosome-treated sides following energy-based treatments. The authors reported particularly notable changes with microneedling plus exosomes, but the small sample size means the results should be considered preliminary.
3. Hydration
Skin hydration is another outcome frequently measured in exosome studies.
In the 2023 randomized split-face study, objective measurements showed greater improvement in hydration on the side treated with microneedling plus an adipose-derived stem-cell exosome-containing solution compared with microneedling and saline.
Improved hydration can make skin appear:
- Smoother
- Plumper
- More luminous
- Less crepey
However, hydration measurements should not be confused with permanent structural rejuvenation.
A temporary improvement in skin hydration is not necessarily equivalent to increased collagen or long-term reversal of skin aging.

The study found significantly better Global Aesthetic Improvement Scale scores on the exosome-treated side.

No serious adverse events were observed.
This is stronger evidence than an uncontrolled before-and-after study because the comparison was performed within the same individuals. Nevertheless, the study was relatively small and short. It should therefore be viewed as encouraging evidence rather than definitive proof.
What Did the 2026 Research Add?
The evidence base is expanding.
A 2026 retrospective analysis evaluated 40 patients who received microneedling combined with an exosome-containing skin booster. The investigators reported favorable patient-reported and aesthetic outcomes, with mostly mild transient adverse effects.
Because the study was retrospective and lacked a randomized control group, however, it cannot establish how much of the observed improvement was caused by the exosome-containing preparation versus microneedling itself, natural variation or other factors.
The newer 2026 randomized split-face study involving radiofrequency microneedling and a topical extracellular-vesicle preparation provides additional evidence of a possible incremental benefit, but its unblinded design and 12-week follow-up also limit the conclusions that can be drawn.
Taken together, these studies make the subject scientifically interesting—but they do not eliminate the need for larger, well-controlled trials.
What Is the FDA Status?
This is one of the most important questions patients should ask.
Microneedling Devices
The FDA has legally authorized certain microneedling devices for specific uses, including improving the appearance of facial wrinkles and facial acne scars in appropriate patients. Authorization applies to particular devices and particular intended uses—not to every microneedling device or every possible application.
Exosome Products
